Community Care Family Medicine cares for all ages, including newborns, pediatrics, and adults. Our family doctors, nurse practitioner, and staff are dedicated to providing the area’s highest quality health care services for you and your entire family. As board certified, licensed family physicians, we are trained to diagnose and treat a wide range of acute and chronic medical conditions for people of all ages. Our staff works with you as a team to provide each of our patients with “whole person” health care. Comprehensive care is key to our goal of providing complete wellness.

Community Care Family Medicine has been Recognized by the National Committee for Quality Assurance (NCQA) as a New York State Patient-Centered Medical Home (NYS PCMH). What We Do

Located in a multi-specialty medical facility on Route 9 in Clifton Park, our practice offers numerous advantages over a traditional primary care practice. In addition to our extended hours of operation, we have surrounded ourselves with many subspecialties and services that would otherwise be unavailable at a standalone practice. This results in better care for you and your family in terms of convenience, service, hours, and on-site subspecialty consultation.

By taking the time to get to know each patient beyond their immediate clinical needs, we are able to offer our patients customized health management services. The result? Our patients typically benefit from healthier outcomes over the course of their lives.

Medical Marijuana

We think you may benefit from Medical Cannabis if you have conditions such as cancer, ASL, HIV/AIDS, Parkinson’s disease, MS, epilepsy, neuropathy, PTSC, sleep impairment, anxiety, and chronic pain, among others. Your first step is speaking with a certified practitioner. Dr. Eric Schnakenberg is a certified provider of medical marijuana. Schedule an appointment to see if medical marijuana is right for you.
