Making an Appointment
You no longer need to call our office to make an appointment. Community Care Family Medicine offers an online patient portal, called mycareDOT™, powered by FollowMyHealth™. You can request appointments through this secure, HIPAA compliant system anywhere, anytime from the internet.

CCFM does offer a secure patient portal, mycareDOT™, powered by FollowMyHealth™ to connect with our office for general messages, appointment requests, prescription renewals and some lab and test results. Contact us for more information or if you’re interested in enrolling.
Please note that email is not a secure medium. Any communication via email should never be considered secure. Sending patient information via email, including clinical or personal information, is not recommended.
Community Care Physicians does not communicate patient information via email, and is not responsible for any clinical or personal information sent by the patient, patient representative, or other party through email. The patient or end user, including but not limited to any individual or entity representing the patient, is accountable and responsible for any content delivered to Community Care Physicians via email.
Canceling an Appointment
If you must cancel an appointment, we ask that you do so at least 24 hours prior to the time of the appointment. We are happy to reschedule your appointment for a better time.
